Referrals are required by some insurance companies. Unless you are an emergency room patient, you may need a referral from your primary care physician in order to become an OANI patient. After you have that referral, please call us and well be happy to schedule an appointment at your convenience.

In
preparation for your first appointment,
please arrive 30 minutes early so you have time to fill out the
necessary paperwork. You must bring your insurance card, a copy
of your referral and any x-rays.
Questions OANI staff will ask during your first appointment:
- How were you injured?
- When did your symptoms begin?
- What medications do you take? Do you have medication allergies?
- What surgeries have you had?
- What is your medical history?
- Are you off work? How long? What work do you do?

If you need to cancel an appointment, please do so at least 24 hours in advance.

Fracture Fee are global billings. Initial Fracture charge then no charge for follow up visits for 90 days (except for x-rays).
